$500 ASDHA Student Scholarship Award Application
The purpose of this $500 scholarship is to provide financial assistance to a senior dental hygiene student. One student in Arizona will be chosen to receive this scholarship.
| Criteria |
| 1. Be enrolled as a senior dental hygiene student. |
| 2. Submit an application and write a brief essay that answers the question "Why should you become a member of the ADHA?" |
| 3. Exhibit outstanding leadership ability in SADHA or participation in state or local dental hygiene association activities. |

ASDHA Student Leadership Awards
These awards recognize outstanding leaders in each of the six dental hygiene programs in Arizona. One applicant from each school will be selected to receive this non-monetary award.

| 1. Be enrolled as a senior dental hygiene student. |
| 2. Demonstrate the highest ideals of dental hygiene professionalism. |
3. Exhibit outstanding leadership ability in SADHA or participation in state and local dental hygiene association activities. |
| 4. Two instructor recommendations must accompany application. |
